Activation fees are for new lines or phone upgrades. If you don’t have a new line or didn’t upgrade through ATT, no activation fee.

I upgraded from an iPhone 6s to an iPhone X. The phone was sent for because they didn't have it on the AT&T store at the moment. I know someone who got the iPhone X through the Apple Store, upgrading from an iPhone 7, and activated it online. Is AT&T going to charge him for the online activation on the next bill?

Maybe. If his phone came with a sim card already set up with his number, then he will pay an upgrade fee. If he got a phone with no SIM card, no fee.
