I’m sorry you are having trouble with submitting a device unlock request online, I would be happy to share with you common mistakes while filling out the form.

All requested information on the unlock request form is in regards to the Account Holder:

• Please fill out your name (no nickname)
• Address
• Last four digits of SSN to reflect exactly as it states in your AT&T account.
• Make sure that you put the phone number to the line that used the phone.
• If you do not have an account passcode (different than password), leave “password” blank.
