iPhone 11 volume control buttons only work with ringer volume, not for ca.ller voice

On my new iPhone 11, I can adjust the ringer volume using the left side buttons.  But there is no way to increase the volume of a voice that is talking to me via cell call.  Left side buttons do not work for that.  Why?

So then last night, I was watching a video and was able to use those left-side buttons to increase the volume on the video.  BUT, they still don't work for increasing the volume on a phone call, incoming or outgoing!  Is it supposed to be that way and if you want to hear the person you have to use the speaker phone???
