If a ch 7 bankruptcy was filed and dismissed on an AT&T account, how can one establish an unlock code for a device previously on account?

You can't. Bankruptcy keeps AT&T from collecting its debt, but it doesn't keep AT&T from remembering it. If a phone was on an account with unpaid debts due to bankruptcy, AT&T still won't unlock the phone without repayment.
