Unable to apply my ACP benefit to my AT&T prepaid wireless

I have been approved for ACP and I already have an Application ID number. I'm trying to register the benefit with ATT prepaid with this form: https://attprepaidebb.com/ and I'm entering the correct data. I also called National Verifier to make sure all data are correct. 

However, on the last page, every time I hit Submit button, I'm getting the same message over and over again: "Oppps, something went wrong, please retry your submission:

Now I'm stuck with AT&T ACP - I'm officially approved for ACP, but I cannot apply it to my ATT Prepaid account due to some failure on the https://attprepaidebb.com/ web form. Please help!

Try to get it resolved by contacting the FCC who sponsors the ACP program. It borders on fraud for you not to be able to use approved ACP benefits.

Thanks for your response! I filed a complaint at consumercomplaints.fcc.gov today. Please let me know if any phone number or email a aila le at Fcc that handles the ACP complaints. Cheers!

Well, thank you Marilyn, but I contacted 866.986.0963 multiple times already and the agents were not able to resolve the issue, they said they don't know what's wrong with https://attprepaidebb.com/ and why Submit button doesn't work. They also said they cannot link my Application ID from the ACP approval to my Att Prepaid wireless because they don't have access and it's an FCC program. The only access is via https://attprepaidebb.com/ form and it doesn't work.  Still this is Att webpage, right? And it doesn't work right? And if you look in the past messages in this forum about ACP for Att prepaid you can see other people are experiencing the problem too. So how you can really help? I'm happy to work with you and whatever it takes to show you how by entering the correct data in https://attprepaidebb.com/ form, the submission goes wrong with the same error message asking to resubmit again. This is clearly a web page issue!
