Typically the phone will prompt you to enter the PUK when it's needed. If it doesn't then try to look up the user manualy for your model and it may tell you how to do it.

Best way is to ask AT&T CS, either by phone 1-800-331-0500 or via chat here on the site, they should be able to look it up.

I would recommend that instead of engaging airplane mode you disable data while roaming and forward all calls unconditionally to your voicemail. That way you won't have any crazy roaming charges but will receive all your incoming texts for free as sandblaster pointed out.

You will get 4G LTE service in the areas where it runs on those bands. AT&T does not indicate which bands are in use in which areas, so it's hard to tell whether it'll work where you use it the most. In short, it's safer to get a phone that supports all 4 AT&T bands.

You can move your SIM freely between these phones without having to call AT&T as long as the SIM card they use is the same size. Since Lumia 920 is using a Micro-SIM and Galaxy S is using a regular (Mini) SIM, you'd need either a new SIM or an adapter. It should although it is potentially possibly that it's too new and not sufficiently backwards compatible to work in such an old phone. But chances of that are pretty slim.
