After inserting the other carrier's SIM and powering up again, you need to swipe the screen to unlock the phone first and only then will you see the dialog for entering the unlock code.

After reading the paranoia in this thread, I was surprised that AT&T instantly gave me the unlock code when I called Customer Care at 800-331-0500. I am only 10 months into my 2 year contract and needed the phone unlocked for overseas travel. I have been an AT&T customer since 2008 under a FAN account.
