Add or replace? Add will require you to select a plan, obtain a SIM and be assigned or port a phone number. Replace is as simple as swapping your current SIM or obtaining a new free SIM from AT&T (best done in-person at a company store).

AT&T's pricing and plans favor multiple users and/or heavy data consumers. If that is not you and cost is a concern, you might be better off looking at another prepaid option like GoPhone or a mobile virtual network operator (MVNO) like Consumer Cellular, both which use AT&T's network.

Since the 6s Plus is the newest phone model, it is best to have Apple repair it, either in-person at an Apple Store (by appointment) or by sending it to Apple Repair. If you are able to take it to an Apple Store, the price may be less than the published $149 (for a Plus), at their option.

AppleCare+ for iPhone 6s and iPhone 6s Plus is $129. If you can confirm that your new phone is in fact covered once you receive it, I wouldn't pursue the matter further.

This worked in the past: Press down the Home and Power buttons until phone shuts off. Launch iTunes and connect the charging cable to your computer. Press and hold the Home button. Connect the charging cable to your phone. Continue hold until iTunes says your device is entering recovery mode (see below).
