First, great job on the troubleshooting you have done so far. Helps to eliminate different possibilities. Have you tried getting the RG replaced? You also mentioned you have a switch that you are using, can you outline the network topology that you have?

It sounds like you have changed a lot of settings on your RG and so I would first suggest that you reset it back to factory settings to make sure you are back to a baseline. Then follow the instructions below for how to properly setup your DMZ+ mode. SOME of the functionality of xbox may work but situations like he posted about not being able to join a game a friend is hosting is a problem you run into when your NAT status on Xbox Live is Moderate.
