And be sure not to mis-enter your PUK code too many time, or you may get permanently locked out of your SIM and need a replacement.

You'll want to contact AT&T to see if the 1st attempt got credited to your AT&T account. If it didn't, then wait a few days then check your credit card account and see if the duplicate charge is still there. If it is still there, call your credit card company to dispute it.

If the SIM card isn't AT&T, that could be the problem. What happens when you insert an AT&T SIM in this AT&T-branded phone?

Interesting issue. Is there any accompanying notification, like in the notification bar? Does it happen all the time, or just while on calls, or just while not on calls?

You probably need to contact the carrier that the phone is currently locked to, to obtain the subsidy unlock code.

To add to Liz's suggestions, also select the option on your phone to Disable Roaming Data (or similar wording). Just in case regular cellular data accidentally gets enabled, that option should shield you from data roaming and the subsequent charges.

Also make sure your computers/desktops/laptops are free of malware. Maybe there's a keylogger on your machine grabbing your passwords as you type them. To check for malware, use several of the available free scanning services out there. Just 1 simple program (Norton, McAfee, etc.) won't be enough.

I've heard of this happening to others. I think on AT&T phones, the hotspot app first checks with AT&T to get "permission" to enable tethering. And if you're international, then the AT&T network isn't availble. This does seem to be a software oversight. Unlocking the phone really should enable tethering without needing AT&T's OK.

It sounds like you're referring to the number/duration of rings before the call is forwarded to voicemail? You may be able to adjust it with a "secret" GSM code entry. I have some old notes that show (and I just tested) that I can check (not change) the duration by dialing: *#61# My phone reports 20 second delay before voicemail. This is an account-based setting for that ...

I hope AT&T will in the (near) future, make more apps disable-able on their phones. I disable a lot of bloatware apps, but not everything allows me to (such as the Email app).

I agree with Wild Banchi. It sounds like the OP is seeking the subsidy unlocking code, not the PUK code. The PUK is for enabling the SIM after it's been locked out by a bad PIN (I think). PUK: You should be able to get this online yourself, within your AT&T account/login. I'm pretty sure I saw it there at one point recently.
