Taking my line from a family account to my own single account

I have recently split from a long term relationship with my girlfriend of 2+ years and I’m on her account with 4 lines. I would like to take my phone off her account and have my own... how would I go about that?

I am a authorized user on her account. Can I do it without her making a call too? I’d rather do it without her calling first.

She has too. Only the owner can authorize a change of ownership
