Switching plans and phones

I currently have an iPhone 3GS and am paying entirely too much for it. In order to change phones and plans do I have to go in to the store or can this be done online?

Interesting. I wonder if a phone bought from Apple would trigger a fee...

From what I know, Apple does not have additional fees like some of the other retailers.
