I would contact customer service or return to the store where you purchased the phone/SIM for assistance.
Customer service's phone number is 1-800-331-0500 - you'll need to choose "more options" at the first menu prompt and then repeat "customer service" at subsequent menu prompts in order to reach a live agent. If your local store is convenient, I'd go that route as you may need a replacement SIM, plus that way you can ensure everything is working properly before you leave the store.
