I have 12 Mbps and want to upgrade the speed, which means you just have to unthrottle the connection. So why does that cost $99? I did a self install before and I don't think this needs new equipment or anything.

The increase to 18, 24 or 45 Mbps require a higher connection profile than is normally configured for self-installs (normaly 13.5, if I am not mistaken). These higher connection profiles have higher requirements on the line and thus all require a technician visit to be sure that the entire line from the VRAD to the NID is of sufficient quality to carry the demand, and possibly to install a new home run from the NID to the RG (if needed).
The pay off is not immediately having to call a tech out to find out why your line keeps disconnecting.
