Thanks for the post and the screenshots.
It appears there have been iCloud issues affecting some customers. Apple is aware of the problem and according to the Apple Support Systems Status dashboard - the issue was resolved recently.
Please try to create a new custom notification, and check back using apple.com/support/systemstatus/ if you still experience problems. We appreciate your patience!
