$90 rebate after 90 days paid service

I was presented with an offer of receiving a prepaid Visa card with activation of Auto pay as well as paying the bill for 90 days. I am over my 90 days and still haven’t gotten anything about it? How do I go about getting the Visa card? I’ve been looking forward to this please help me if possible. Thank you .
