family map - adding phones

i am trying to understand the family map limitations. we have three separate accounts. i started by adding the first account (a family plan for my son), the script does not allow any other number to be added. do all numbers have to be on the same billing account, or worse - do all numbers have to be in the family plan?

I have 3 phones on one account and have used the family map feature on my cell for over a year. Approx 2 months ago the app stopped working on my cell. I have deleted and tried to add back the app but can't find the app anymore in the App Store (I have an IPhone). The app works fine on my laptop.
