How to obtain passcode to my old iPhone 6?

I have been an AT&T customer for 30 years.  

I have an old iPhone 6 which I no longer remember the passcode for.  I want to give it to a family member.  Also, I don’t know if it’s unlocked.  

I no longer have the original box so I can’t get to the IMEI number.  Any suggestions?
