Data on Verizon phone

Anyone have any suggestions on getting data to work on an unlocked Verizon phone? It was working fine and them just stopped. I've checked the apn,.changed and entered it again and the data just won't work. Tried nxtgenphone, att phone, etc nothing fixes it 

Data works fine with an att branded phone so it doesn't seem to be a network issue. Seems like a phone issue but I've never been stumped like this before and not gotten it to work 

The crazy part is it was working and just stopped and never came back
