how do I retrieve my network unlock code

I need help unlocking my phone outside the US.  Last week I moved from Louisville, KY, USA to live in Jordan for 6 months.  Yesterday I purchased a Jordanian SIM card for my phone.  Today I tried to operate my phone and was asked to enter a "network unlock code."  I thought the unlock process was completed before I left the US.  Before I left I changed from a monthly pay plan to a prepaid plan so I could keep my US phone number when I return.  I think I may have accidentally deleted an email with the network unlock code without knowing what is was.  I do still have my "unlock request number".  I don't think the "network unlock code" and the "unlock request number" are the same as the latter includes letters and the request for the former on my phone shows only a numbers/symbols keyboard.
