The Ethernet requirements depend on the type of speeds you're looking to get, and how you're planning on using the internet. It is recommended to use Cat 5e or Cat 6 Ethernet and compatible jacks or above for wiring, so that you can get the best performance for your available bandwidth. Those cables will be able to provide you with up to 1000 Mbps speeds if you are eligible for 1000 Mbps service at your location.

Yep, cat5e or cat6.
In regards to the ends:
RJ45 if on fiber account (red port on modem) [must use all 4 pairs in the rj45 end]
RJ11 if on copper (green port on modem) [must use 2 pairs if on bonded pair service, wouldn't hurt to just use 2 pairs anyways]
And if you're just running data around (copper or fiber), say for a hardwired computer, etc. You'd make rj45 on both ends.
