Mobile Data Logs
1) Does the timestamp on this log show the actual time the app was opened or data was downloaded?
2) If the Phone is connected to WIFI when data is downloaded or an app is opened, does it still show under "Mobile Data"?

The data logs in your online account are not in real time. The only real time record of data usage is on the phone itself. AT&T data logs record data when it's posted which could be 3 hours to 3 days after actual use. Data is sometimes batched and sometimes posted in small amounts. Each Tower you connect to while you are away from home will record an amount of data which is reported back to AT&T and recorded in your online account. You will find similar information in the bottom of your usage logs in your online account information.

The information provided by @formerlyknownas is correct. In fact, part of those details can be found under For data plans with overage charges when you visit Understand how wireless usage is billed.
As already explained, the mobile data that you are seeing is just regular data usage. Text and data usage are combined in both categories.
Use AT&T Device Support Center to see the apps using the most data. After selecting your device's make and model choose System followed by View & Manage Data. Follow the instructions that are provided.
