cancel service for death

Gail [EDITED] Acct #[EDITED]passed away October 24, 2020. Please cancel her service. Ms. Gail was afflicted with MS and lost the ability to send and receive phone calls several years back and in the flurry of treatment and care, her phone service was not cancelled then and her account has been auto deduct for several years with no phone service. While her estate doesn't expect to recoup any of those funds, we do request that the auto deduct provision is immediately terminated. The estate will gladly send a death certificate if requested. The address on the account is[EDITED], Liberty, SC 29657. Please attend to this matter immediately. We feel the account has been receiving these funds for the past several years and absolutely no service was being used.

This request has been submitted by Helen [EDITED] Caregiver for Mr. Ross[EDITED]
