Access to iPhone with Visual voice mail is part of your price plan. You can log in to your account online and cancel the pro-tech insurance.

There should be a $30 upgrade fee as well as your regular plan charge. You might also look to see if you are being charged a $5 fee for 'next up' . Next up allows you to upgrade early. If you are willing to stick out the full 30 months without upgrading early you can remove the $5 fee as well as the insurance. 

I guess you don’t review your bill very often. As noted, the “Access for iPhone 4G LTE w/ Visual Voicemail” charge is part of your plan and has been there for as long as you have had the plan. Two insurance charges? I’ve never heard of that one.

@sandblaster the protect advantage insurance that's about $14+ a month is broken down into two separate charges on the bill.  One about $8 and the other one about $6.  Part of it is Asurion insurance, the other is support, cloud storage, additional benefits like unlimited battery Replacements and lower screen replacement cost.
