puk codes, My mom's acct clearly pays my ph # as well as hers but only her device shows up in support

My phone is on my moms acct and when I look up the unlock puk codes I only see her device although the bill clearly shows she pays for my phone number as well as hers.

 

Why dosent my phone show up? I cant find anyway to get my puk code and have only 4 chances to guess the 8 digit PIN (which Ive NEVER had an 8 digit PIN for anything in my life). Please advise??

So after an amazingly brief ph call to ATT we got it unlocked. I had to let them speak w the acct owner a few times which was understandable but all n all pretty painless
