Have you visited Apple forums for suggestions?
Your iCloud and Apple account are the same and should have the same password.
Have you maintained your Apple/iTunes account? If so, you use your Apple ID and password all the time, how did you forget it?
Did you change your email address, and not change your Apple ID in the correct manner, and now cannot login to your Apple ID on the phone? Do you still have access to the email address?

Did you change you iTunes password, but iCloud doesn't recognize the password?

more details please....

Hello @derrick-313

If you purchased your phone from the AT&T store, they should have a receipt on file, you'll just need to stop by the store to get a copy.

Thanks,

Dmitriy

What if you purchased the iphone online directly from ATT? Would ATT still have a record online or would you have to go to an ATT store to get it?

What if you purchased your phone from an AT&T branch store, and not an AT&T corporate store? Are the affiliated at all, because it's been just over a year since I bought my last iPhone and I'm not sure they keep records for a receipt of purchase more than a year and need to have Apple to remove my iCloud account from my old iPhone I sold to someone else and forgot to erase the iCloud settings when I restored the phone.

You can remove it yourself.  Login to your iCloud account on another device and delete the old phone.   Per Apple:

icloud.com.  Log in with your Apple ID and password.

If you're using iCloud and Find My iPhone on the device, sign in to iCloud.com or the Find My iPhone app on another device, select the device, and click Erase. After the device has been erased, click Remove from Account.

I tried having the person shut the phone off so I could go into find my iPhone to erase it and remove it from my iCloud account so they could use it, and I did and it looks like it was removed but when they turn the phone back on I'm still seeing the phone under find my iPhone.

Turning the phone off is NOT part of the directions.  In fact iCloud tells you the phone will be erased next time it connects to wifi or cellular.

It can't be removed from your iCloud if it's off.  It requires a network connection to remove the phone.

I purchase a phone through ebay and I am experiencing the same ordeal. The person who sold it to me forgot to erase icloud settings and says that the device does not show in his icloud anylonger.  Apple is asking for a proof of purchase. like an actual receipt not something from ebay or amazon.  I ask the person who sold it to me to go on a live chat with me and we can both unlinked the phone to its original settings so he can be sure I dont use his password or his account in a negative manner. Its the best for both parties to do this.  I guess he does not want to give out his pass words for personal reasons. But what if he cant get an actual receipt from AT&T
