Getting repeated messages that access card (745) cannot be read

I have been getting daily "access card (745) cannot be read." It requires removing the access card, reinserting it, then pressing reset. Whatever I was recording at the time  (52 minutes worth) said it had recorded, but then I got a message on a black screen saying all or part of the program did not record. No playback was possible.  I don't care as much about the not recording, but don't like getting the message and having to reset my tv each time. I don't think I have a U-verse TV, but am not sure.
