Need to return Uverse equipment

I just had to switch from AT&T Uverse to Direct TV.  The installer told me I have to return the old Uverse equipment but I can't find instructions anywhere for returning it.  Is there an address to return it to, do I have to ship it and if so how and where, etc.  Can anyone assist?

Take it to your local UPS store and have your Uverse account number. They will ship it for free and give you a tracking number to hang on to.
