Help! I've tried every APN setting and still no 4g!

11-14-2017 I bought a new 2013 Sony Xperia ZL C6506 online, and got a new SIM from the local AT&T store. The phone has all of the frequencies (bands 1,2,4,5,17) that AT&T uses for broadband data.  I've tried numerous APN settings but the phone only shows the "H" and speed test results are pitiful.

When I hold the phone to my ear I can barely hear the person on the other end but they can hear me fine. Od
