Unlock device portal

Why are my requests to unlock my cell being cancelled? I have submitted 3 request so far and confirmed all requests and 24 hours later I receive a cancellation message that I did not confirm. I have a screen shot of my last confirmation attempt. Please fix or create an easier way to unlock devices.
