It's not what a PUK code is. A PUK code is from the carrier whose SIM card you are using. It locks the SIM card to a particular phone so that your service can't be stolen. If you are using a T-Mobile SIM card and being asked for a PUK code you contact T-Mobile. If you are using an AT&T SIM card then you need to contact AT&T.
If you have not carried unlocked your phone and it is an AT&T phone then you request carrier unlocking, not the same as PUK, here.

Do not mix up your codes. Do not accidentally enter a PUK code when being requested for a carrier unlock code. If you permanently lock your phone to AT&T, AT&T cannot provide you with an mck code they don't have it. And if you permanently lock your PUK code you have to go to your service provider and have your sim card replaced

This is the code that was given to me and it worked. Not sure if it will work for you but you can try it.

Unlock Steps - Use Non-AT&T SIM Card
1. Power off the device, remove the battery, and insert a non-AT&T SIM card.
2. Re-insert the battery and power on the device with non-AT&T SIM.
3. The device displays \"Enter the device unlock code.\"
4. Enter the eight-digit unlock code.
5. Press OK.
Note: You are allowed five attempts to unlock before your device permanently locks to the AT&T network.
