reactivating my sim and wireless device

So I have an at&t sim card which I've been using for like three years now.

I deactivated for like 4 months.

I would like to reactivate my sim card but don't know how to.

 

I tried using the website but I don't know my username and passwords........

In addition, there is at&t store close to my place. (5hrs away)

A deactivated SIM card can not be reactivated. You will need a new SIM card.

I am still confused because I was able to reactivate it last summer after deactivating it.

Did the system change?
