Note 4 Issue
When the phone is plugged into charger (using original fast charger and cord) it charges very slow and in battery menu says connected to usb. When I take phone off charger it will continuously switch to charging although nothing is plugged into phone. The only way to remedy this is by taking battery out and putting back in. I've tried everything I can think of, looking for anymore ideas before I head to the store. Thanks

Sounds like a connector/port issue. You can try vaccuming around the plug in case it's just dust/fuzz in there, but more likely you've got a bad connection/solder/other bad hardware thing going on.

Please try running and testing the device in safe mode for 2-4 hours. Safe mode temporarily disables third part apps. To exit safe mode just reboot the device. Here are the steps for your device: http://bit.ly/1fSmIVC. If the device works normally in safe mode then a third party applications is the likely cause.
