ATT web site error in Unclock request page

I am trying to unlock my Atcatel 4952R flip phone. I meet all the criteria. I enter my data but the site rejects it as incorrect - but I verified it against my profile and it is correct.

SO I tried to unlock a second phone (not yet eligible) but get the same reject error on my personal info -

This strongly suggests that the web site page https://www.att.com/deviceunlock/ has an error.

It seems impossible to report this error to get it investigated and fixed.

Hi Lynn,

do not know what you mean by the Message Icon.

I have an Alcatel flip phone  4052R for over 12 months. I have moved to Canada and need to use a Canadian supplier and sim card. My account is pay as you go and is in credit.

When I go to your page

https://www.att.com/deviceunlock/

I supply the information - first/last name, email etc and submit

It takes me to

https://www.att.com/deviceunlock/unlockstep2

and gives me the message

The information you have entered is incorrect. Please update and try again.
However my information is correct, as I logged into my account and verified the data
I have a second phone - Galaxy A13, but have not had it for the required 6 months to unlock it - but I tried the number any my info and got the same error message on that account too.

I received an email from Customer Care on the 15th of March that my unlock request was being investigated - it is now the 29th and I have not heard back with any progress.
