Received unlock instructions but no unlock code
I sent out a request to unlock my iPhone 4. Within an hour, I received an email: Congratulations! Your request was approved. Instructions to unlock included but there is No Unlock Code.

I am so sorry to hear after receiving your approval for your device unlock, it didn’t come with the code. Don’t worry though; you have come to the right place, and I would be more than happy to assist!
The great thing about unlocking an iPhone, is it doesn’t require an unlock code such as an Android would. There are two different ways to unlock an iPhone; either by using a new SIM Card, or by completing a Backup & Restore on the device.
If you have a SIM from your new carrier, please follow the steps below:
If the above does not work, or you do not have a new SIM available, don’t worry you can still unlock your device. Just follow the steps below to unlock it:

erased it using Setting + General + Erase. Then I setup the phone and
received msg "SIM Not Supported", it seems to me that the phone is still
locked. Please let me know what did I do wrong, and how to fix it. Your
help is very much appreciated. Thank you !

I followed your instructions in the email but I got the following failed messages:
- From Iphone : "SIM Not Supported ........."
- From PC's ITunes: "The SIM card inserted in this iPhone does not appear to be supported.....or request that this iPhone be unlocked by your carrier...."
Below are steps I did:
1. Since the phone has no data so I do not need to perform a backup. I erased all old data: Setting>General>Reset>Erase All Content and Settings.
2. Connected IPhone to PC and started Setup the phone and received above messages.
Please let me know if I missed any extra steps and what do I need to do ?

@Winter32708 Since the iPhone 4 has an older version of iOS, I'm pretty sure the only unlock instructions that will work is to connect your iPhone to iTunes and either do a backup/restore or reset to factory settings but you need an ATT SIM card in the phone when you do that. The SIM card does not have to be active but it does need to be an ATT sim. Once the phone re-authorizes in iTunes, you then should get the message the phone is unlocked. At that point you can put in a SIM card from another carrier.

inactive AT&T SIM card) to iTunes and did a reset to factory settings. It
seems to me that everything are accepted by iTunes (old phone number and a
lot of info were display) but I did not get the message the phone is
unlocked. Please explain more what do you mean "Once the phone
re-authorizes in iTunes" ? Thank you !

@Winter32708 It's been a long time since I unlocked an iPhone 4, I don't remember exactly where in the process that message displayed but it is very possible the unlock completed successfully but no message displayed. Go ahead and try the other carrier SIM card.
