PUK code is available on your online account or by calling AT&T customer service.

The PUK code is for the SIM card. If you are not using an AT&T SIM card, you do not need an AT&T PUK code. You need the PUK code for the company whose SIM card is in the phone.

did the unlock code provided unlock your phone? if it did and your phone is blocked, it may be blacklisted (reported lost or stolen) and will never work.
If the unlock code provided did not unlock your phone, you can private message @ATTMobilityCare with your contact information and concern and they will assist you.
