misc byod all is not my phone type, so I can't use insurance

When trying to file an insurance claim for my iPhone X, it gave me two options instead: another phone on my plan and "MISC BYOD All" with a glitched photo of a generic phone (not my iPhone).

My iPhone should be eligible for replacement/repair, but this MISC BYOD All raises an issue with the system.
