Which Username and Password to use?

We have HBO Max through our AT&T internet account. However, we do not know how to access our Smart TV with DISH. When it asks for the Username and Password, we have tried the Username and Password we use for AT&T; but, these do not work. What Username and Password should be used for accessing HBO Max?
