Who do I report a broken sidewalk utility box cover to?

I have reported a damaged telecom concrete sidewalk cover to the City of San Rafael, CA on two occasions now and they have told me they handed it off to AT&T. It has now been over three months since moved in to our home. This is a safety issue for anyone walking on the sidewalk and we are close to a school. Seems like it would be a priority to get this repaired.

Boken sidewalk cover in lawn area near the front of 4886 Saginaw Cir., Pleasanton, CA 94588. This was reported before to the call center (800) 288-2020. They were unsure of how to take the report since I do not have an AT&T account. Five weeks later a cover was placed over the box but it was the wrong size so it keeps falling into the box. It has been three months now and would appreciate it if this could be resolved.
