Record only new, but records all/old programs?

All of our many programmed recording schedules are set to "record only new", but many programs are recorded multiple times (same episodes) and even old/past seasons. Some programs I can completely delete the setting then set up a new one and it fixes the issue. But some record duplicates anyway.

Some of it is the channels not providing the correct guide data to the guide maker the other is how you setup the series recordings, using search will use its default settings of all channels and On Demand, using the guide and pressing record twice will only record that channel and time unless the guide data is wrong.

Dish had similar settings and rarely had such a problem. Even when set to record only on a specific channel, it will record on demand episodes as well.
