At&t rewards card

We have had direct TV installed since September 3rd... I was informed when setting up for new installation that I would receive a $150.00 reward card. They sent me an email and I claimed it and it shows it was in the mail but I not received it and have called about it a couple times and still nothing... I was going to use it to pay my direct TV bill... What do I need to do to request another card???
