Tjohns2, I called them to have mine done and they were very helpful! HOWEVER, it's supposed to be reflected on this bill and they took the full amount anyway. So, now I have to call again. Just a forewarning for you. Hopefully, yours will be done correctly! All I needed was my benefit code/number and they pulled it up. If something changes after I call them, I'll update this! Good luck!
