ATT Fiber Signup Rebate Card Not Received after 3 months os service

Hi,

I had opened a AT&T Fiber internet account on June 10 2019. I am yet to get the 100$ rebate card. I had captured screenshots while applied for this offer. I tried to chat, no luck.. called 800 288 9983 , but put on hold for almost 2 hours. Could you please help in resolvving this issue ?
