Return uverse equipment

I have been trying for months now to return my uverse equipment. I live over an hour away from a ups drop off, where they box it for you. So I was told they would mail me a box and I could mail it back, I never got the box and have called 5 times and still nothing. I have asked for a label where i can just ship it back and they won’t give me one of those either. What do I do?
