easement consent form for fence in Palm Beach County

I need to obtain approval from AT&T for an existing fence that encroaches on a utility easement. I am located in West Palm Beach, FL. There is no phone number available to call and speak to someone.  Looking for assistance to get the ECF completed.

All utility companies signed off on easements and att is the only company without clear guidelines of how to obtain easement consent from them. I have called multiple numbers to various departments and still had no luck. Could someone advise of the process? I need email or fax # to send in the application. Thanks
