Why is my Reward Card disqualified?

I signed up for new service with AT&T in part because of the reward card offer. I recieved my letter and claim number. When I put the number in, it tells me that it is disqualified.  I have called the Reward Center several times only to remain on hold for over 45 minutes. This is so frustrating.
