Confused about start date after ordering

Ordering online should be simple, but it was very unclear.   I kept expecting the form to ask me for the start date.  No where did it do that.   How do I define a start date?  I don't move until a few days before the end of month.  I don't want to pay for service that I can't use yet.

 

Also, there were many address fields involved and no order confirmation summarizing the options I provided after the fact.  Specifically billing address was confusing to me as I'm not sure if billing location is the same as install location.  I hope not, because I put my current address for billing since that is tied to most of my CC/bank accounts at the moment.   Is there any other place to review the order I just put in?
