That is fiber connector not copper phone jack.
Your looking for a fiber coupler for SC/APC to SC/APC
to my knowledge only comes with the bracket…

Pic would have been nice…
If standard wall telephone wall jack with green cover would be RJ12/14.
If this is an ATT Branded Jack (ATT on wall plate) this is a self contained unit requiring the entire unit to be replaced.
A damaged jack replacement should be a billable ($99) service call.

This is all thats there .. im missing the connector that the end of that green piece connects into and mounts on the inside bottom of the face plate
