Thank you so much for that input. I would have definitely gone the SIM swap route based on your suggestion. However, I did the tech support chat and this lady agent (Andrea Cardenas) helped me troubleshoot and actually made it work. It was all in the reset option under General. One of the Reset options was Reset Network. Once I did that it took about a minute for the LTE to show up. Kudos to Andrea. And thanks for you guys suggestions as well.

First, insure there is LTE coverage available in your service area.
Second, go to an AT&T store and get them to perform a SIM swap procedure.
This is the only thing that finally provisioned my phone correctly for LTE.

Before First, ensure that the "Enable LTE" toggle is on for Voice & Data.
