Are you the original purchaser of the phone? Did you fill out the form correctly as an active AT&T customer? The phone has not been replaced by warranty or insurance?

And the most obvious, is the phone even locked to AT&T. Check in cellular, about phone, Sims status. If it says AT&T it is still locked AT&T. If it says no SIM restrictions it is not locked.

Yes, I did the fill correctly.

The phone has not been replaced by warranty or insurance and yes the phone is locked to att.
